That pattern, slow drainage throughout the house getting gradually worse over years rather than a sudden backup, is the classic sign of scaled cast iron. The bore has closed in from all sides. Descaling with a chain knocker restores it where there is sound iron left, and the camera survey tells us whether there is.

Does cast iron need replacing or can it be cleaned?

It depends entirely on how much sound wall remains behind the scale. Where there is good iron underneath, descaling is far cheaper and less disruptive than replacement. Where the scale is holding a perforated wall together, descaling will open holes, and we will tell you that rather than take the work.
